ELAS
(Endoluminal Ablation of the Saphenous vein) this procedure is for the patient with venous insufficiency. This is determined in our office by Dr. Hrdlicka. He will perform a Duplex ultra sound. If reflux is found, then an ELAS may be considered necessary. This is a minimally invasive procedure that is also done in our office as an OutPatient procedure. With ultrasound guidance, Dr.Hrdlicka uses a needle to canulate the incompetent vein, then passes a guidewire through the needle. A catheter is then positioned over the guidewire. The laser fiber is then placed in the catheter. The laser is then fired 100 to 144 times at 1 to 2 second durations as the laser fiber is slowly brought down out of the vein, closing the vein down. A 3-layer wrap is applied and worn for 48 hours after the procedure. Then the patient will wear compression hose for 2 weeks. The patient may resume normal activity any time after the 48-hour period, but may plan to refrain from work for one week.
Tumescent Anesthesia has been used most commonly by dermatologists and plastic surgeons for years. Recently, it has been applied to other procedures, particulary vein surgery. When Dr. Hrdlicka administers the tumescent anesthesia, the tissues are infiltrated with the dilute liquid anesthetic. The large volume of fluid causes the vessels to be compressed which results in very minimal bleeding. This anesthesia has a prolonged effect and also helps with any postoperative discomfort.
